Alzheimer Society of Kingston The Alzheimer Society of Kingston offers support and education for families of people with Alzheimer's (and related forms of dementia). We also advocate on behalf of those with Alzheimer's and their caregivers. We ensure they have a voice when it comes to public policies that affect them. The Alzheimer Society of Kingston also raises money to help find a cause and cure for the disease. Alzheimer Society of Thunder Bay The Alzheimer Society of Thunder Bay exists to alleviate the personal and social consequences of Alzheimer Disease and related disorders and to promote research. Alzheimer Society – Victoria County The mission of the Alzheimer Society Victoria County is to alleviate the personal and social consequences of Alzheimer Disease and related disorders and to promote research. Baycrest Centre A multi-level health-care system dedicated to improving the quality of life of the elderly with a strong focus on brain functioning and mental health. They provide help to more than 3,000 older adults every day – from the well and active to the ill and cognitively impaired. They also support and educate families, conduct research into aging, teach students and help train those who care for the elderly. Bethammi Nursing Home The home supports a full range of programs and services, providing care to elderly, physically-disabled adults, those afflicted with Alzheimer's and dementia as well as the frail with high-intensity medical needs. Catholic Family Services of Hamilton A voluntary family agency that has served the Hamilton and Burlington communities since 1944. Services are available to people of all faiths. Central Community Care Access Centre The Central Community Care Access Centre (CCAC) connects people of all ages to the care they need at home and in the community. It also helps people find the right housing options when living independently is no longer possible. Central CCAC’s catchment area includes more than 1.7 million people who reside in South Simcoe, York Region, North York and parts of the Toronto communities of Etobicoke and York. Christian Horizons An Evangelical Christian organization offering a full range of support to individuals with a Developmental Disability across Ontario. Coleman Care Centre A 112 Long Term Care bed facility located in Barrie. Community Care Access North Simcoe & Muskoka Community Care Access Centres (CCACs) provide an access point to health and personal support services to help individuals live independently at home or to assist them in making the transition to a long-term care home. Community Living Campbellford/Brighton A local association for community living with a record of accreditation with distinction and service excellence; the association has received funding from the Ontario Trillium Foundation to conduct a study of the needs of older adults with developmental disabilities. Community Living Central Huron The goal of Community Living - Central Huron: “That people live in a state of dignity, share in all elements of living in the community and have the opportunity to participate effectively.” Community Living Chatham-Kent Community Living Chatham-Kent is a non-profit charitable organization started by volunteers and parents of people with an intellectual disability who wanted to see people living and contributing in communities. Elisabeth Bruyere Research Institute – University of Ottawa The Élisabeth Bruyère Research Institute is a partnership of the SCO Health Service and the University of Ottawa. The SCO Health Service is a health care organization affiliated with the University of Ottawa that actively promotes the integration of research and education with service delivery in its four programs-Complex Continuing Care (348 beds at Saint-Vincent Hospital), Long-Term Care (71 beds at Villa Marguerite; 198 beds at Résidence Saint-Louis), Palliative Care (36 beds), and Rehabilitation and Care of the Elderly (98 beds including the Day Hospital). Fairmount Home Fairmount Home is a non-for-profit municipal 128 bed long term care facility with a strong reputation for dementia care. Families Matter Cooperative An On-line Resource Centre that Connects people with developmental disabilities, their families and friends to each other, to up-to date information, to comprehensive lists of resources. They facilitate Initiatives that empower our members such as: Supportive Housing Options, Social Enterprises, Other initiatives needed by members Community Living Georgina A non-profit organization that offers support and services to approximately 150 individuals with developmental disabilities who live in the town of Georgina. Haldimand Association for the Developmentally Challenged The Association provides individualized support to individuals to live in a variety of residential settings and teaches life skills, budgeting and social skills. Helps individuals to participate through work, recreational and leisure activities within their communities. Haliburton, Northumberland, Victoria Access Centre Simplified access to a wide variety of community health services for eligible clients. Services are directed at minimizing hospital admissions, decreasing length of stay in hospital, as well as minimizing or delaying admission to long-term care facilities. When the latter is no longer possible the Access Center facilitates the placement process. Halton Support Services Provide the initial contact point for families/individuals who are interested in accessing residential and non-residential supports from the Children and Adult Developmental Services Sector in Halton. L'Arche Toronto, Daybreak and Ottawa An international network of faith-based communities creating homes and day programs with people who have developmental disabilities. Lakeside LTC The Centre provides a homelike environment and high quality care for individuals who are unable to live independently and require 24-hour nursing care. Lakehead University – Thunder Bay Ontario Lutheran Community Care Centre The Lutheran Community Care Centre is a Social Service organization of the Lutheran churches in the Thunder Bay Area. Established in 1979, the Care Centre provides an array of services that are dynamic - changing as the needs of the community change. A common theme among all our services is a holistic approach and a focus on advocacy especially for disadvantaged individuals. Mary Centre Mary Centre provides residential services to adults with a developmental disability in Toronto, Brampton, and Woodbridge. Qualified staff assist each person to reach their full potential. Marycrest Homes A non-profit, interdenominational Home for the Aged, Marycrest is home to 156 residents who require nursing and personal care. Muki Baum Association A non profit organization that runs residential and day programs for children and adults with both an emotional and developmental disability. North East LHIN One of 14 Local Health Integration Networks that have been established and launched in Ontario. North West Community Care Access Centre The North West CCAC helps people to: Live independently at home; Apply for admission to a long-term care home. Ontario Association of Non Profit Homes and Housing for Seniors (OANHHS) A provincial membership-based organization that has represented not-for-profit providers of services, care and housing for seniors for over 80 years. Ontario Agencies Supporting Individuals with Special Needs (OASIS) Facilitates the sharing of ideas, resources, systems and information and liaises with Government on behalf of member organizations to improve the development of cost effective quality supports for individuals with developmental disabilities. Ontario Association of Community Care Access Centres (OACCACs) Serves as a collective voice for the contributions made by CCACs who provide a simplified point of access to Long Term Care for more than 400,000 people each year. Ottawa-Carleton Association for Persons with Developmental Disabilities The Ottawa-Carleton Association for Persons with Developmental Disabilities' (OCAPDD) purpose is to support community integration and personal well-being for children and adults with developmental disabilities within the Regional Municipality of Ottawa-Carleton. Participation House Provides residential housing and day programs to adults with varying levels of physical disabilities within York Region. Progressive Therapy Progressive Therapy provides physiotherapy services to long term care homes, and the North East Community Care Access Centre. Providence Care (PCCC) is the leading regional provider of non-acute health care in Southeastern Ontario and a major centre for teaching and research, through our affiliation with Queen's University. Reena Provides a full range of support services to people with developmental disabilities within a framework of Jewish culture and values. Reena was instrumental in the formation of the Ontario Partnership on Aging and Developmental Disabilities. Regional Support Associates Regional Support Associates is a group of dedicated, caring specialists in developmental disabilities. Regional Support Associates provides services to adults with a developmental disability. Their services are offered to the individual, their families and agencies supporting them in their community. There are no fees for their services provided they are within the catchment area. They are associated with the Woodstock General Hospital. Rockcliffe Long-Term Care Facility Rockcliffe specializes in long-term care for older adults with varied health and functional needs. Royal Ottawa Hospital The Royal Ottawa Hospital is the provider of specialized mental health services in Eastern Ontario for people with serious, complex and recurring mental illness. Rygiel Supports for Community Living Rygiel supports for community is a non-profit organization governed bya volunteer Board of Directors. Much of our funding is provided through the Ministry of Community and Social services. The remaining comes through charitable contributions. Our mandate directs us to support children and adults in our community who have a developmental disability as well as other handicapping conditions. The agency also supports children, teens and adults who have an intellectual disability and believes that everyone can contribute to their community. St. Joseph at Fleming This Long Term Care Home, situated on the campus of Fleming College, believes that care and compassion are essential in fostering sanctity of life and in creating a home environment that promotes personal worth and dignity. The partnership with Fleming College supports a unique opportunity for cross sector learning through student placements. Surrey Place Centre A interdisciplinary, community-based agency committed to enabling people with a developmental disability and their families to enhance their quality of life through integrated service, research and education. Surex Community Service Surex was founded in 1980 as a pilot project. The pilot was initiated by the province in order to assess the feasibility of severely and profoundly handicapped individuals living in community homes. With the success of the pilot the province began to plan for the closure of institutions in a broad and encompassing manner. Since then Surex has grown through the province’s many community initiatives and now provides residential and day supports to many individuals with a developmental disability. Surex operates 11 group homes and a multi service day program in the eastern part of Toronto. They also provide support to many aging individuals and have worked effectively with several long term care programs in transitioning their seniors to the long term care sector.
